Community Health Equity Survey
Take the Community Health Equity Survey:
To improve the health of everyone in Massachusetts, we need to center the voices of people and communities who have been long denied an equal chance at health. The CHE survey is an opportunity for residents to share their experiences and priorities to help shape the future of our health. The Massachusetts DPH will use survey results to improve programs, make decisions about funding and resources, and support policies to improve health inequities. They will also share the results of the survey with partners across the state to support the important work they are doing to improve health in their communities.
The survey will be open from July 31st 2023 through early Fall 2023. Anyone aged 14 or older can take the survey. It takes approximately 15 to 30 minutes to complete and is available in 11 languages: Arabic, Cape Verdean Creole, Chinese (simplified), Chinese traditional, English, Haitian Creole, Khmer, Portuguese, Russian, Spanish and Vietnamese. The survey will also be available in ASL later this summer. Just click on the link above to get started!
